OUR PROGRAM

Welcome to our T.I.M.E. Diabetes Management Program!

In this program, Community Health Workers educate individuals with diabetes to understand and overcome common community healthcare barriers.  This program is based on monthly in-person classes with patients.  These classes are made up of four 30-minute sessions and a healthy meal at the end of the class.  In those four sessions they will have one large group 30-minute session with all patients together.  Then the group is divided into three small groups and are rotated through social topics (practical ways to overcoming barriers), behavioral (overcoming mental/spiritual barriers) and a time with the medical team.  In this curriculum we share ideas on questions that you can make your patients through weekly calls and that relate to the information shared in the monthly class.
