Welcome to our SIMPLE Diabetes Management Program!
In this program, Community Health Workers educate individuals with diabetes to understand and overcome common community healthcare barriers. This program is 100% remote. It is designed with monthly videos that can be sent to patients monthly. In this curriculum we share ideas of questions that you can ask patients on a weekly basis and that are related to the video of the month.
